Surveys and forms: what you can make

Use surveys for feedback, registrations, simple requests, polls, and quizzes.

Zotonic's Survey feature is a form builder. You can collect feedback, take registrations, ask visitors for information, or create a quiz without writing a separate application.

A form is a page with questions, settings, and saved responses. Its editor has four tabs:

  • Description: the title, introduction, and thank-you text.
  • Settings: answering rules, result display, and email handling.
  • Questions: the fields, answer choices, and pages people complete.
  • Results: submitted answers, charts, and downloads.

For a first form, start with a short registration: name, email address, and a yes/no question. Test it before sharing its address.

Your site must have Surveys enabled, and your account must be allowed to create and edit surveys. Ask your website manager if the Survey category is missing. File uploads and connections to another system need additional attention; do not assume every form stores uploaded files or sends data elsewhere.

You are responsible for the questions and what happens to responses. Before building, decide who will read them, what follow-up respondents can expect, and when the form should close.
